What we need to review a referral
1. Current plan of care or person-centered plan
2. Diagnosis and level of support needed
3. Funding source and current status
4. Guardianship status, if any
5. Reason for the referral and desired timing
6. Any history relevant to placement or roommate matching
What happens next
We review the referral and respond within one business day. If we can serve the person, we arrange a visit. If we
cannot, we say so and tell you why, so you are not waiting on us while you could be looking elsewhere.
Housing without services
Some of the people you work with need somewhere to live more urgently than they need a service provider. The
homes are owned by Blue Ocean Equities LLC, a separate company, and a person can rent without receiving any
services from Better Day. A tenancy is a tenancy — it does not depend on who provides someone’s services, now
or later.
